Join the Memorial Hermann Healthcare Analytics team as we describe our process for creating an environment to conquer the Quadruple Aim. In order to be successful in the population health arena, organizations must meet objectives as outlined by the Institute of Health to improve the patient’s experience of care, improve the health of populations, reduce the per capita cost of healthcare, and improve the provider’s experience. Memorial Hermann is recognized as a leader in population health management with much of our success derived from the utilization of people, process, and technology. Without analytics, the ability to identify areas of improvement and design your process to meet the Quadruple Aim is nearly impossible. This session highlights the technology, the people, and the process utilized by Memorial Hermann to bring together disparate data to improve costs, outcomes, and the patient and provider experience, as well as a increasing covered lives while improving reimbursement.
