Incorporating Social Determinants of Health into EHRs: Experience from the Field

Join national thought leaders from Kaiser Permanente and OCHIN at the Oregon Chapter of HIMSS Population Health Management Forum as they discuss their research on, and their organizations’ journeys incorporating social determinants of health (SDOH) into Electronic Health Records (EHRs).

With the awareness that SDOH - the non-clinical circumstances of patients' lives – can impact health more profoundly than clinical care there is a growing national emphasis on systematic documentation of SDOH in EHRs. However, there is little scientific evidence nor are there national guidelines on how healthcare providers can or should integrate SDOH into their EHRs and workflows.

The forum is open to HIMSS members, guests, and all those interested in learning more about SDOH and strategies for incorporating them into EHRs and workflows. Individuals who will find this forum beneficial include: informaticists, clinicians, pharmacists, healthcare analysts, administrators, Population Health Management leadership, and Social Determinants strategists. Bring your questions for this educational networking event, all attendees can share insights and ask questions in this interactive presenters/audience discussion.

Speakers

  • Rachel Gold, PhD, MPH, Lead Research Scientist at OCHIN, Inc. presenting results from her team's research on strategies for using EHRs to document patients' SDOH in community health centers, and approaches to helping primary care clinics adopt SDOH documentation
  • Briar Ertz-Berger, MD, MPH, Assistant Director of Operations for Special Populations, Kaiser Permanente, discussing Kaiser Permanente’s journey to incorporate SDOH into patients’ care plans.

Continuing Education Hours

This event has been approved for up to 1.5 hours in continuing education (CE) hours for use in fulfilling the continuing education requirements of the Certified Professional in Healthcare Information Management Systems (CPHIMS) and Certified Associate in Healthcare Information and Management Systems (CAHIMS).
